A large should fit well if you want it to be snug, as the pic shows. If you wants it to hang loosely go xl. As a 36ddd gal M is snug but still comfortable. Hope this helps.

Q: Does the shirt come with the straps at the neck or does the model have something on under it?

The model has something under it. No straps are on the shirt.

Q: I weigh 158 & I wear a 38c. What size would you recommmed ?

I ordered the large and easily could have ordered the medium. I normally wear medium - my weight is anywhere from 145-155, and my chest is 38DD with Victoria's s bras and 36d with Fredericks bras. I washed the shirt in cold and dried it on the low setting and mine did not shrink. I don't want to send you in the wrong direction and then you're not happy with the size you ordered, but for me, I should have ordered the size I normally wear, medium.
