How do you keep the pads from falling off

They should not fall off if properly fitted. You do have to press down firmly to get the pads to lock back into place. Make sure you only press at the outside edge - fingers on the back of the earpiece and thumbs on the plastic at the edge. Never press inside the earpiece or you may damage the speakers.
