Do you recommend buying an additional waterproof cover or will the one that comes with it be enough?

Aways have a spare one. You will need to line dry the cover that comes with it, but when kids are sick they can puke for several days (sometimes couple times a day)

I washed the mattress protector cover once in cold water on gentle and air dried it out. It now won’t fit or zip up over mattress! Anyone else?

Didn’t happen to me I washed immediately after purchase. I would contact babyletto!

Can I use a mattress protector on top of this?

You can but I don’t believe it is necessary as the cover included is removable and washable.
