Q: Is there a drain in the middle?

There are 4 potential drains - 3 bottom drains - one on either end and one in the middle or you use the adapter to drain from either side of the channel drain.

Q: Does this item come with two end caps or one cap and one outlet?

It comes with a end cap and an outlet connection ( example, if you order 3 units, you will have one end cap and one outlet connection ). (be sure to pour a concrete footing under the drainage trench for best results..set the unit into the wet concrete, letting the unit settle a 1/4" or so..)

Q: is there install instructions somewhere

I did not find installation instructions anywhere, however it's pretty straight forward . Just lay it out before you join the various sections if you have more than one.
