Replacement tray - will a 4 inch replacement dish fit this warmer?

The inner diameter size of this wax warmer is 3.54 inches. so your replacement tray bottom size should be less than 3.54 inches

This isn't melting the wax melts nor pieces broken up. I tried the extra bulb but there's no difference. Any suggestions?

Doesn’t make sense the lit bulb warms the wax melts all the time without any problems

What size light bulb is it G40 or G50?

Unfortunately the bulbs that come with this aren’t labeled with bulb size - just 120 V, 25 Watts. Hope this helps!
