Hi Paul, I made the Large size. They appear to be what I would consider a normal width. But, as they are leather they will stretch when worn. Not sure if this helps, but I do not have them with me to measure. I know the person I made them for has a wider foot and they seem to do fine.

Q: Is the sole cushion attached (glued in place)? Or, is it loose and just slides in once lacing is complete?

It's somewhat attached. I've gotten a couple that the sole wasn't quiet in the right spot but they don't slide around.

Q: Is there a kit that comes in children's size?

This is the smallest size Silver Creek makes.

Q: Has anyone used leather lace to make these and have a suggestion of what lace to buy on amazon?

I used the nylon weave lace from the kit. I used leather from a kit I 1960; that was harder.
