Q: How does the bar come out to put the roll of toilet paper on?

The rod for the toilet paper is not stationary. It just sits on the brackets on the end. You just pick up the rod put it through the paper and you are Ready to go.

Q: How far apart are the screw holes? If possible I would like to use the same holes that are there.

Those are not screw holes from left to right. They are just curls in the design. The one screw hole is at the top in the middle and I came down two and three-quarter inches and put another screw in that triangle

Q: How does the toilet paper holder attach to the wall? There is only one obvious place for a screw but the item was shipped with 3.

I put a second screw in the opening directly down from the first screw which is behind the tp roll when installed. It holds really well and hasn’t moved at all in over a year.
