Excellent Resource!
I am a Behavioral Health Specialist (LCSW) working in a community based family practice medical setting in Maine. We are in the beginning stages of working to become a Patient Centered Medical Home practice and I have found this book to be a valuable resource. Considerations are presented in an order and manner that is very useful. I have shown it to many professionals that I collaborate with and used it as a helpful go-to reference. The book contains many ideas for: assessment tools; communication with medical professionals; "marketing" oneself within the practice setting; collaboration efforts and; improving efficiency & effectiveness; working with medical professionals within fast-paced medical settings; etc. This book is very helpful to me as I transition from a traditional psychotherapy setting to the Medical Home model within an outpatient medical setting.

Good book!
I have not had the opportunity to finish using this book but what I have read so far is very insightful and I will be able to use for practical applications.

Great resource for any social worker new to working in the Primary Care setting
I have been working in the social work field for 10 years; however, just started a new job working in the primary care setting. I found this resource to be invaluable as I navigate this new position. Having access to the worksheets online has also been a major time saver!! I highly recommend this book to any social worker who is new to the primary care setting.

Love it
As a neophyte BHC psychologist I was looking for an guide to the this new field in order to transition my skills over. This book is great! The layout of the book is well thought out, very informative while still being concise. Highly recommended for those who want an intro w/ how to advice.

Very informative; I highly recommend this book for anyone just starting out in integrative care. Easy to read and good examples provided of how to implement behavioral health in a primary care setting. Sign up for the web site.
