Almost perfect!
I really like have this compact book for report sheets! It has a spot for everything I typically write down in report. My only complaint is, there’s a not a big enough space to write down events. Many patients have been here for multiple days to weeks and I have to try to cram in daily highlights somewhere, but I make it work. Definitely worth the price! Highly recommend!

Good idea
I love the concept of the compact report sheet - however the past medical history and current situation are personally to small for me. I think the back side could be made into better use

Great idea but writing fields are far too small to fit all necessary information for many patients, such as the elderly with extensive health history. Used one shift and decided to buy another type.
