Q: Can you use it in bed or on a couch without the straps?

We do not recommend you use this cushion in bed. It was meant to be used on a chair.

Q: What's the measurement from base of back to top? I'm 5'5.75" and want something to hit my shoulders/neck areas.

I am 5'5". I have my Shiatsu on a solid wood chair. I find that if I move my body into different positions I can hit most of the spots that need it. The main massage areas are 22" with a gap of about 7" in between. The Shiatsu chair's back is too heavy to position it higher on my wooden chair with the strap. I like my massage chair even though I have to move my body around. I hope that helps you.

Q: Do you need a special chair? Is this model the best for neck pain

Different chairs affect the strength of the massage. A straight-up kitchen chair worked the best for me. It worked on my soft couch, but it did not massage as deeply. The angle of the chair affects how it sits on your body. If I were to do it again, I would order the back massager and the neck massager separately for a better fit.
