Does this actually create a stream to spray up in your nose? Or do you practically have to hang upside down?

It sends a gentle spray, but it is not like a fire hose. I inhale through my nose when I use it; you will have to determine for yourself if that's right for you. By the way, I love this stuff!

Can this be used in a nebulizer?

This is to be used as a nasal spray (just like it says). I would NOT put it in a nebulizer unless a doctor says it's okay & I don't think you'll find a doctor that would okay this.
