Does ink bleed on the paper?

I use pen in my book and it doesn't bleed at all. :)

does the book come with the pen in the picture?

It does not. At least mine did not have a pen, but then again I’ve got lots of cool pens that I’d rather use. Also, the book is gorgeous, you won’t be disappointed.

Can the pages be refilled?

No. The person who told you yes was incorrect. The description of the journal clearly states this as well.
