Can this pump be used outside during the winter with temps averaging 20 degrees?

Yes, I put mine in a 5 gal bucket with a top on it. Cut a hole in the side for plug and cord. Put a brick in the bottom to keep it out of water if some water gets in. All my fish lived. Put a block of styrofoam foam in pond with holes filled three to support air stones. Buffalo NY

Does all 4 output have to be used?

No, either plug some off or as I did just put two together with an adapter and some air hose

Can this run continuously

I have been running it for about a year continuously without a problem. It is a bit noisy.
