This works really well for my debriefing at the end of clinicals. I write neat so it’s not hard for me to get everything I need about my patients.

Not practical for practice
As others have stated, there’s not enough room for everything you need to write in a day. There should also be a mobility category. And I wish it fit in the pocket of scrubs. The tall version of this is even smaller so it just doesn’t work. It should be one pt per page, not two

Great tool for healthcare providers
Nursing report made easy: Includes room for the Pt name , Code status, allergies, admission date. Head to toe assessment, & labs, etc. FYI: Book arrived slightly bent. Overall, the blemish was insignificant.
