Why is the water not as hot coming out of the rainfall head?

It should be the same temperature coming out of either head. If you started the water on one setting and then switch it to the other, the water sitting in there won't be hot and will take a second to flow theough. Did you check it with a thermometer? Different parts of the body experience heat differently, so it's probably just your perception.

Why can't this item be shipped to my address? Why would Amazon even have this item if it cannot be sent?

I have never had this problem. If you have only a post office address, I would suggest giving the physical address of the post office. Tricky, but it works

The know that you use to switch from handheld to rain head fell off and fell down the hole of the drain. How do I get another one ?

You have to contact hibbent customer service. I cannot recall the e-mail address, but you can find it on the internet or on the sheet that came with installation instructions.
