Will it work upside down?

It should, but the buttons on the remote would be reversed. If you pushed the left pan button the camera would pan right

Can the head be permanently plugged into power and will it stay awake, looking to mount on perch 20 ft off ground.

It runs on a lithium battery. I haven't tried running it while it is plugged in. Just know you are not able to program it to pan or tilt on a timer. If you are not pushing the button to make it pan or tilt it will remain stationery.

Does it have a built-in spirit level?

I don't know what you mean by spirit level but it has 9 different speeds for pan and tilt. You can hear a little humming noise from the motor when recording.
