Q: Where is this Moleskine made?

Communist China. I have a Leuchtturm 1917 notebook which frankly I like better than any Moleskine notebook I ever had. They are made in Democratic Taiwan. I note Lenchtturm also offers planners if China is distasteful to you. I have used Moleskine planners for years but because of this very issue I may try Lenchtterm next year

No the date is not stamped on the front cover

Q: Does the planner include a removable address book? I had a Moleskin planner years ago and liked that feature.

Not any more, probably because most people use their phones as address books now. But it has a handy pouch for notes in the back, and I think it's a great planner. I get it every year.

Q: How many lines pages are between the monthly layouts?

There are 2 pages between the Monthly. The back side of one Monthly and the back of the next month.
