Can these be used more than once or are they single use?

They are multi-use. I am not sure how many times you can use one before it needs to be disposed of or cleaned. I suppose it depends on how dirty your glasses are each time you clean them and whether or not you use a liquid cleaner as well. I am certain you can wash the cleaning pads a few times using dish soap and plenty of warm water then patting the pads dry with a paper towel. I think you could get a lot of use out the 12 cleaners.

Since 6 lens-cleaning devices are pictured, am I to assume "12 pcs." means the customer is to assemble them to make 6 cleaners as pictured?

There are 12 lens cleaner no assembly is needed. Each one is individually wrapped.

Do not waste time nor money. Wash the glasses and forget about these things. They do not work. Your fingers will do a better job.

Can these be washed to remove grime so they continue to work well?

I've rinsed mine under the sink with soap and they work fine.
