Q: Can I attach the DEWALT DG5132 Heavy-duty Yoke-Style Suspender tto this belt?

It can be done but it will take some creativity. I used 1/4x20x3/4 bolts to attach mine.

Q: What level do you wear the belt? Meaning do you wear it at the normal pant waist level? Or higher like for back support?

That's pretty much a personal choice. Just make sure you are comfortable and familiar with the placement of your tools. Use logic, weight and the amount of tool use dictates where to mount and ware your tools. Just make sure the things on your belt don'the interfere with work.

Q: Does the 5 inch pad have velcro ??

No. The five-inch pad, which falls at the small of your back, does NOT have velcro. However, there is velcro at the front of the belt, on the inside tab behind the buckle area, which helps secure the belt and allows you to make those critical minute adjustments. This is a GREAT belt, very comfortable. It helps distribute the weight of your tools and makes all-day wear very comfortable. Get it!
