You would have to be very gifted using hand tools to get it to look great.

can this be installed to a wall with 2x3 stud (2.5") ?

Not out of the box, it can't. This product comes with a full size outlet box that goes behind the angled outlet plate. That box, when attached makes this a tight fit on a regular 2x4 wall (3.5" of cavity space). You would need to find a different outlet box that is much shorter.

Does it include a bottom access plate (to get the media wires through the wall to TV) Or its just one plate and I need to purchase another separately?

No, it does not. You only get the 2 plates here (1 for media wires and 1 for the outlet). The additional plate that the wires will travel from is not included.
