Does it stick to ceramic

The adhesive is so strong I believe it will even though I did not put my own there.

What is the diameter of the roll bar and the "head" at the end?

The diameter of the roll bar of the toilet paper holder is 0.75 inch and the “head” at the end is 0.97 inch.

Has anyone successfully moved this from a painted wall and reattached it, if so how

I had to reattach after the adhesive failed. I used a small screw and drilled a hole thru the base. I was attaching to a wood surface but you could use a drywall insert.
