As I recall, the box was longer than the article; the width was about as wide. I believe the additional length was due to the mirror. The was a critical element as it allowed me to read and watch tv which made being face down bearable. You may want additional packing if you are shipping outside the US so it arrives in a useable condition.Sorry, I do not have exact measurements

Does the head piece attach to the frame by velcro. Can the direction be changed so that it sits sideways?

Yes, the part where you place your face is attached to the frame with velcro. Although I never tried it, I believe you can slip the u-shaped bar under a mattress or firm couch cushion to have it going sideways. In my opinion this is the best piece of equipment if you need to be face-down for any length of time. It saved my sanity.
