is this a level 3 or level 1 mask?

Box information on the back states "meets ASTM F2100-11 Level 3 requirements." Says same info on front and top panels of box.

both sides of the mask are white which side is the outside?

I believe you can tell which side is the outside by where the straps attach to the mask. The inside of the mask has the straps attaching to it. Also, the longer metal piece goes across the nose- the shorter side under the chin.

The box looks different from the previous 6 orders. Did the manufacturer change the print and picture on box? Could this be a knock off?

How many plys/layers?

There are 4 layers. Plus a metal bar to bend over the bridge of the nose and under the chin. For a very secure fit. The mask is worth the money for the protect it provides. I have seen this mask in the dental practice I go to.
