Does this have a backing to it that covers both the bottom and back? Or is it just beads? I’m worried about dimples once I remove it.

A foam like pad for the bottom portion comes with it.

How does it attach to the seat?

It slips over the back and rests on the edges of the seat back. Mine does not have the headrest attachment.
