Does this come with the envelope-sizing tool that's shown in the video?

No unfortunately not. There is a scoreboard and envelope punch by Bassein as a kit and I think if you turn the board over the envelope sizes are on the back. The one I bought was just the punch.

What is the size (radius) of the corner cut?

If you measure 0.5cm from the square corner, then that's where the curve begins. Hope the video helps to make it clearer.
