Does this pen have the option to buy refills?

Yes indeed! I bought a 2-pack of refills when I purchased the pen, item #1800702..I actually just ordered another pen as a gift. It's clearly stated "Refillable" on the package.

Does this pen run out of ink easily?

I haven't had any problems with it running out of ink. I like the pen enough-- not my all time favorite pen, but I like it enough.

Does the pen start reliably and immediately? And is the writing smooth without any skips?

This pen is more of a marker style pen. Think of a very thin Sharpie. The writing is smooth and starts immediately. Of course, that fades over time as the pen dries out. However, it does last a good long while.

Any one else's clasp too tight for the shirt pocket? It ripped a hole in mine.

I don't clasp it on my shirt. So not sure
