It works in two directions, you can crank items up and then switch a lever too crank down. As it it a hand crank the speed is controlled by how fast or slow you turn the crank so I guess you would call it variable speed.

Because I am using mine to help me lift items from the ground level to the deck of my split level home, I used lag bolts and bolted it to the top rail of my deck. What you would attach it to would depend on how you plan to use it. Best of luck !
