Will this fit a 6’5” tall man?

I am 6 ft tall it adjust for me , a strap could be added to your weed machine or if it not going to fit send it back.

Will this work with a stilh pole saw?

No not if it's above your head ok for hedge triming

Will this fit a 230 pound woman?

Well, it fits me and I'm 350 pounds, so I'm sure it will fit. But I could be wrong.

Can the pad be swapped to the left side?

I took my as delivered right handed padded trimmer and was able to move everything for left-handed operation, but it took me about 40 minutes to make the switch (you’re welcome). Now another 40 minutes to switch all the straps and padded shoulder strap back for right hand operation. It is worth the effort for this quality product.
