Should I get a medium if I'm usually a small in american sizes? Also, how tight is the shirt?

I would. It’s fairly true to size. But, I don’t like too tight in the tummy area.

T-shirts are spandex and tight fitting. If you wear a regular in a Hanes or Gildan shirt size, what would be the appropriate upsize?

I wear XL size shirt regualy, I ordered this in the 3X and it fit perfect, not to tight, not too loose. I would go 2 to 3 sizes up depending on how tight or loose you like it. Material is stretchy and nice and soft.
