Q: There are no pictures of the back. How is this mounted on the wall?

I have the floor cabinets but they also mount to the wall to prevent tipping. There are slots cut into the back pieces wide enough for a bolt to go through it. So there are wall anchors and the bolt just goes through the back and into the wall. No fancy wall mounting. I don't have this specific one but I imagine like the other 2 I purchased its probably the same.

Q: Is there a way to make door open the full 90 degees instead of sloping downward while open?

I didn't install the supplied shocks but the door itself does open 90 degrees if not a little more
