What is the width of the collar?

Sorry about my original comment. Roughly width round is 1 cm/ 10 ml, and flat is 2cm/ 20 ml

"medium 45 cm long with holes starting at 36 cm" - is 45cm the position of the last hole or the actual length? my dog's collar (neck size) is 43cm.

You may be struggling with this size of collar, the collar is 45cm in length, and the last hole is about 4cm from the end of the collar. you do have room to add another hole which may just fit. I bought the collar for a Labrador whos neck size is 40cm and I fasten it on next to the last hole if this is any help, It is a nice comfy collar if you get right size. Hope this helps in some way.
