Q: Does it stops the signal from Line Out when you connect a headphone or can you have both a headphone and line out at the same time?

There is a switch on the back that will cut off the RCA pass through when headphones are plugged in, or you can have both headphones and line out working at the same time. The pass through is just a pass through though, not a pre-out. Volume control has no effect on the signal going through the pass through.

Q: Any chance this can power Fostex t50rp mk3 (modded) headphones?

I don't have anything remotely as hard to power as the t50rp's. The reviews that I've seen and read all seem to say that you need like 4-8 watts to drive them properly. The max power handling of the T50rp's is lower but it's not about using all available power so much as having the added headroom. But I've also seen people saying that they're driving t50rp's with Schiit Magni 3's and this amp has about the same power as it does. You could give it a shot. It's a great amp if you put the Burson V5i opamps into it. It's actually a very very good amp when you do that. But horses for courses. For a T50rp mod I think many would recommend something like the Emotiva BasX A100, the AudioGD NFB1 or NFB11, or the Massdrop THX AAA 789.

Q: is it portable i use my headphones in the hospital; etc maybe nx2 topping ?

The A30 is not really portable. It's not very large, but it needs to be plugged in. If you're looking for something portable, the NX2 would be better. FiiO products are also really popular portable DAC/amps.
