Do the case come with the air freshener ?

If by case you mean what is seen in the photo encasing the air fresheners, yes!

Does the fan or air conditioner of the vehicle have to be in the ON position in order for the product to smell? Or does the product smell without them

The air conditioner does not has to be on in order for the product to smell. The smell stays inside the vehicle even with the AC off.
