Q: Would this work to affix a bunch of hotfix rhinestones to clothing?

It works just like a flat hot iron, except it doesn’t get real hot. Don’t know if this will work for your purpose, but I would not recommend it....a real disappointment!

Q: The box says 110 - 220 volt, is there a switch to go between voltages?

It's only 110 looking at the plug connector that can't handle 220 and you would never need 220 that's the voltage for a stove or or electric clothes dryer which uses a completely different plug

No the heat is not adjustable on the one I have.

Q: Will it work for ironing baby clothes?

This iron is approximately 4 inches long, I imagine if you really wanted to iron anything with it you could. But when I took it out of the box I literally thought it was a child's toy.
