Can you buy extra gel packs?

I don't know , but I would think tat you would just buy another heel pack since the gel packs must be most f the cost.

Is the ice on the bottom of the heel

The ice covers the bottom of the heel and the ankle.

Does it cool where the foot and the leg meet and the foot bends?

It seems to cool everywhere! Very good solid product.

It's a "fits lots of people" size. I wear a size 8 or 8-1/2 shoe, pretty average and it would work fine on a smaller or larger foot. If this is a really large foot or a child's foot, it might not work so well.
