how do you decide which size fits the vitrectomy chair best?

The 8" x 4" is the favorite size for all situations. The other 2 sizes work, but the 8" x 4" is ideal. Any of them are easy to adjust by just pointing the top mirror at whatever you want to see, and look down in the bottom mirror. If / as necessary prop the mirror at the correct angle with a pillow, book or box. Best wishes on a complete and speedy recovery!

How do I read a book?

You wouldn't need the mirror to read a book. You would simply place the book under face and read it with the eye that didn't have the surgery. If I am misunderstanding your question, please let me know.
