Poor tolerances or defective product
The adapter does not work and a 2.5mm plug will not seat properly in the adapter leading to no audio coming through the adapter. All other components that are interfacing with the adapter are tested working. The adapter is either broken or manufacturing tolerances are so poor that it does not function as advertised.
