Will this work for an adult?

I think so as it works well in my costume. I'm 71 years old and six feet tall. However, my wife often says that I'm not very grownup. 

Is there a stick or hook to hold it from the inside?

yes, there is a stick to hold it from the inside. I'm not sure if the one we ordered was defective, but our 5 year old took it out of the box on Halloween and when he held the stick, the ring around the hook popped off and the hook detached from it completely. Unfortunately, in pieces, we were not able to use it for trick-or-treating and ended up throwing it away within 15 minutes of opening.
