Q: does this come with the top (for the tv) and bottom outlet (that plugs into an existing outlet)?? or do i have to buy a separate bottom outlet?

I bought two to serve as an outlet for the TV on top and my accessories below.

Q: Will this fit in a standard 2 gang electrical box? Looks like the angle on the outlet would prevent that. 3 gang needed?

Standard is a single gang. It fits one standard outlet, standard box, comes with box and outlet. Designed to be set between two studs not up against one, held by flip clamps to the drywall.

Q: Are the 120Volts conductors completely enclosed in a box, as per the NEC?

Yes there is a PVC on the back of the receptacle cut out. I wondered the same thing. Mine worked out very well.

Q: If mounted directly to drywall (not studs), is a low voltage mounting bracket needed?

No bracket is needed. Cut out a hole in the drywall, insert and the locking tabs will pull tight when tightened with a screwdriver. The outlet box has a separate back to keep wires contained.
